About Us

Accident Investigation (2)

Accidents and incidents, however large, can have a huge affect on a company and the employees. It is therefore important that accidents and incidents are investigated thoroughly, and without delay, so that lessons can be learnt and control measures implemented to prevent further occurences.

Our consultants can respond quickly to either assist and support you with the investigation process, or to conduct the investigation on your behalf and provide a full report into the findings.

The process of investigation involves

  • Visiting the site of the accident to take physical measurements, photographs, etc
  • Reviewing/ copying all documents relevant to the site, activities and people involved
  • Interviewing and holding discussions with witnessess, managers, specialists, etc

In investigating incidents we aim to identify the factors that conntributed to the incident and detail controls measures needed to prevent a recurrence. Our analysis identifies the range of factors that contributed to the incident.

The type of factors we examine are:

  • People factors - training, experience, activities, supervision, etc.
  • Equipment factors - suitability, condition, design, etc.
  • Environmental factors - weather, work conditions, temperature, lighting, etc.
  • Material factors – products, chemicals, etc
  • Procedural factors - instructions, work practices, systems of work, etc.

A report written report with recommendations will then be provided on the investigation. This report will include photographs, plans, and witness statements taken during the process.

Such reports can then be used in support for defending criminal actions by the Health and Safety Executive or civil actions from employees or third parties.

We can also train persons within your organisation on how to investigate incidents objectively to reduce the possibility of similar incidents occurring again.

'Competent Adviser' Retainer packages
Competent Adviser Solutions to support you when you actually need it, not when you don't ! Let SMC Ltd act as your essential Partner to help you through the field of health & safety.

All organisations, whatever size, have a legal requirement to have a ‘Competent’ Health and Safety Adviser under regulation 7 of the Management of Health and Safety at Work Regulations 1999. In addition, many clients and Contractor Assessment schemes are requiring that organisations have access to professional advice as part of the requirements for tendering.

The perfect solution would be have one of your full time employees take on the role of 'Competent’ person to deal with your Health and Safety management.

However, devoting human resources and the training of personnel to meet this requirement can be too costly for many small to medium sized organisations.

Safety Management Consultancy Ltd have a range of Competent Adviser Retainer packages that are designed to provide a level of support based on the size of your organisation and the level of assistance you require. The aim is to provide you with a level of support you actually need, and what is 'reasonable' for your business.
